Stands for failure modes and effects analysis.

According to ASQ, FMEA is a step-by-step approach for identifying all possible failures in a design, a manufacturing or assembly process, or a product or service.

“Failure modes” means the ways, or modes, in which something might fail. Failures are any errors or defects, especially ones that affect the customer, and can be potential or actual.

“Effects analysis” refers to studying the consequences of those failures.

Failures are prioritized according to how serious their consequences are, how frequently they occur and how easily they can be detected. The purpose of the FMEA is to take actions to eliminate or reduce failures, starting with the highest-priority ones.

Failure modes and effects analysis also documents current knowledge and actions about the risks of failures, for use in continuous improvement. FMEA is used during design to prevent failures. Later it’s used for control, before and during ongoing operation of the process. Ideally, FMEA begins during the earliest conceptual stages of design and continues throughout the life of the product or service.

social networking services are Internet-based applications
user-generated content (UGC) is the lifeblood of SNS organisms
users create service-specific profiles for the site or app that are designed and maintained by the SNS organization and
social networking services facilitate the development of online social networks by connecting a user’s profile with those of other individuals or groups. Most social network services are web-based and provide means for users to interact over the Internet, such as by e-mail and instant messaging and online forums.

FMEA through social networking

in order to perform FMEA, the first step is “Assembly of the FMEA Team”. This team usually interact through social networks. This social networking could be simply getting together in a room (most popular) or interact through the internet. The latter is still in developing mode. LiToChe is an example of such a system.

Physics Chart Check

Regulatory standards, as well as AAPM recommendations, require treatment courses be reviewed before 10% of the dose is delivered.Generally, the initial physics chart check confirms correlation between diagnosis, prescription, and plan, and ensures accurate information transfer between the plan and the RV system. The process also reviews consistency and sensibility, which relies on an experienced physicist’s judgment.

Physics Review Checklist

A checklist is required for properly doing the chart checks. A proper checklist needs  continuous improvements based on the proposed Failure Modes, reported Incidents, and editorial/technical comments. There will never be a final version.

Checklist through social networking

There are many ways a social networking platform can be employed to build and maintain a checklist. LiToChe is such a platform, where the users can 1. use the updated checklist, 2. propose failure modes, 3. Report incidents, and 4. just comment on the tasks.
